In Asia, Nasal Implants are usually the most popular choice. In America, there is a general fear and admonition against implants (to frank disdain) from surgeons here in the West. The reason for this is the real potential for rejection of the implant. This rejection can lead to significant cosmetic issues that can manifest with the extrusion of the implant. Extrusion, is when the implant finds its way through the skin of the nose and is exposed to the outside. This is an obvious great fear for someone considering Asian | Ethnic Rhinoplasty. But in Asia, there are thousands of people with implants with no significant issues. In fact, there are many superb long lasting results that have been achieved with implants in Asia and here in the United States.

According to the most recent studies, the risk of rejection of the implant is rather low in the range of 3-6% (for either silicone [silicone Implantech website], Gore-Tex, or Porous Polyethylene [medpor website] in a very recent meta-study, a study that gathers information from a collection of studies). Many problems that occur with rejection are the due to the way implants are used in the nasal tip area. Many of the extrusions seem to occur either at the nasal tip or inside the nose near the tip area. The main reason for this is that the implants are placed directly next to the nasal tip skin. Usually when implants are placed into the nasal tip, there is pressure being placed whenever you try to increase the height of the nasal tip with implants. Dr Young decreases this risk by using implants or cartilage to project the natural cartilage in order to have the natural cartilage against the nasal tip skin. This significantly decreases the extrusion and infection in this area.

Using Your Own Cartilage for your Asian | Ethnic Rhinoplasty: For some people that are concerned with the usage of implants, Dr. Young can also use just cartilage if that is your desire. However, this could entail using rib cartilage and the procedure can thus be more involved. The options if you were to use your own cartilage are: ear, septum and rib cartilage. Some surgeons even consider, and use, bone from your skull or hip. Dr. Young has used all those different options and can do this for you as well. Many patients do not wish to be involved in possibly such an extensive procedure and, thus many elect to augment their nose using implants.

Explanation of Asian | Ethnic Augmentation Rhinoplasty using Implants:

Below is an anatomical diagram of a nose to introduce you to some of the terms that you may hear in rhinoplasty discussions. Dr. Young typically augments the nasal bridge with an implant. He uses mainly medpor or silicone. He prefers Medpor, given that after 12 weeks, the body incorporates this implant and grows into the implant to allow it to function much like your own tissue. For the nasal tip area as we mentioned before, Dr. Young likes to present the tip cartilages naturally against the nasal tip skin to decrease the risks of extrusion in this area. He does this by using septal cartilage or medpor implants that are sewn to the septal cartilage which then works as a strut of force that the nasal tip cartilages can be sewn to in a more projected manner. Then with the nasal tip cartilages sewn in the more projected position, they can be refined to create the shape of the nasal tip that you desire.

Nasal Bones and Anatomy

If you look in the picture above, the lateral crus and medial crus are the portions of the nasal tip that are sewn on to the strut of cartilage or medpor. This strut is in turn sewn on to the septum in the position that best presents the nasal tip cartilages (that are made up of lateral and medial crus). Then the lateral and medial crus are shaped to refine the nasal tip in the desired shape. Again, the benefit of this approach is to place your natural cartilages against the nasal tip. By doing so, your risk for extrusion decreases significantly.

Below is another picture which shows a blue shape, a visual representation of where the implant of silicone or medpor would be placed to augment or enhance the nasal bridge area. The green shape represents where we would place your cartilage or medpor implant to allow the medial and lateral crus to be advanced onto it to create more projection for the nasal tip and also to help refine it. The green shape actually fits in between the two lateral crus and medial crus and is sewn to the septum (the midline stabilizing structure of your nose). The solid connection of the graft or implant to the septum is essential to allow a stable base for the lateral and medial crus cartilages to be sewn in a more projecting position.

From the side view, you can see that the nose has been lengthened by Dr Young’s technique. The nasal bridge between the eyes has been raised. You can see that in the before picture to the left, there is really no bridge at all. The eyes actually project more than the nasal bridge. The nasal tip is more refined and smaller, less bulbous and round. The tissue around the nostrils are refined as well. This was done with our alar resection techniques. Notice how the nostrils are seen a little more from the side. The tissue around the nostrils is not hanging over the nostrils and the results are a more pleasing appearance in this area.

This oblique view shows much of the same things but from a different angle. This angle allows you to appreciate the nasal bridge from the left and right oblique views. These oblique views allow you to appreciate symmetry between sides that are otherwise not apparent from other angles especially the nasal bridge.

From the frontal view, you can see that the nostrils are less exposed. The tissue around the nostrils are decreased in size. The base of the nose around the nostrils is generally a little bit more narrowed. The bridge is more prominent. The nasal tip is more refined. Another thing to notice is that the incision under the nose in the column of tissue in between the nostrils is hard to appreciate. In other words, the scarring from this incision is imperceptible!

The below pictures show the side by side comparison of an Asian | Ethnic Revision Rhinoplasty procedure at one month. You can also click the link to watch a video of a surgery similar to this patient here, Asian | Ethnic Revision Rhinoplasty. She had her original procedure done by another surgeon in Korea. Her implant had extruded and we carried out her reconstruction in 2 stages. The first stage was to take out the implant and allow the nose to heal in a non-infected state. Then our 2nd stage entailed creating a bridge from ear cartilage and muscle covering tissue. The tip was reconstructed with medpor sheets uniquely designed by Dr. Young which were employed to present the natural tip cartilages as discussed above. Our future plans are to do some more scar revision on the tip with laser resurfacing and dermabrasion / dermasanding. At one month, she still has some swelling at the top of the bridge which should come down soon. We will follow up with more pictures in the near future.
